Request a GP home visit

If you or someone in your family is unable to come to the surgery because of illness, then you can request a home visit.

Please do not use our online triage form to make a GP home visit request.

Home Visits

If you or a family member are too unwell to attend the surgery, you can request a home visit by calling our Care Navigation team on 01793 705783.

Whenever possible, we encourage patients to attend their appointments at the surgery, as this is the quickest way to receive care. Home visits are limited and take more time. In some cases, you may be eligible for a phone appointment.

Other Types of Home Visits

You may also be visited at home by:

  • A community nurse, if referred by your GP

  • A health visitor, if you’ve recently had a baby or are newly registered with a GP and have a child under five

Changing or Cancelling a Home Visit

To change or cancel a home visit, please call the practice. If your visit is no longer convenient, let us know as soon as possible so we can offer the slot to another patient.
