Primary Care Network Explained

A Primary Care Network is a partnership between GP practices that enables individual services to work together to deliver more personalised and coordinated care to patients. By collaborating, practices can share expertise and resources to better meet the needs of their communities.

Brunel PCN3 Ashington House Surgery and North Swindon Practice together form Brunel PCN3. This network, made up of two GP surgeries operating across four separate sites, allows us to provide accessible care that is rooted in the local community and strengthened by the wider network.

About Brunel Health Group Our Primary Care Network is part of Brunel Health Group, a partnership of multiple independent GP practices across Swindon. Together, we care for a registered population of more than 264,000 patients.

Brunel Health Group works closely with a wide range of local health and social care providers to deliver coordinated, person-centred support. This collaborative approach helps us promote wellbeing across our communities and respond more effectively to the needs of our patients.

By working as a unified group, we strengthen the voice of primary care in Swindon and advocate for services that reflect the priorities of the people we serve.

Our PCN Team We provide shared access to our Pharmacy, Physiotherapy, and Wellbeing teams across all practices within the network, ensuring patients benefit from coordinated care and a wider range of support services.

Our Pharmacy Team consists of Clinical Pharmacist, Pharmacy Technicians and Prescription Clerks to delivering in-house pharmacy support to our practices.

Our First Contact Physiotherapist (FCP) is a skilled clinician who specialises in musculoskeletal (MSK) conditions. Their role is designed to give patients faster access to expert assessment and treatment, without needing to see a GP first.

Our Wellbeing Team includes LTC Care Coordinators, who play a vital role in supporting patients living with chronic or long-term health conditions. It also includes Social Prescribers, often known as link workers, who help people improve their health and wellbeing by connecting them with non-medical support available in the community.

As a patient registered with one of the GP surgeries in our Primary Care Network (PCN), your first point of contact for medical care will typically be your own practice.

However, to enhance the quality and accessibility of care, you may also be supported by members of our wider PCN team. These professionals are part of the Additional Roles Reimbursement Scheme (ARRS), which enables GP practices to expand their services by introducing a diverse range of healthcare specialists.

ARRS roles include staff such as Clinical Pharmacists, Social Prescribing Link Workers, First Contact Physiotherapists, Care Coordinators, and more. While they may not fall into traditional clinical categories, they play a vital role in delivering personalised, holistic care tailored to your individual needs.

By working collaboratively across our PCN, these additional healthcare professionals help ensure you receive the right care, from the right person, at the right time.
